ATLANTA, April 25, 2016 /PRNewswire/ -- Wellcentive, a leader in value-based care solutions, today announced the expansion of its population health management platform and services within the growing Affinia Health Network (AHN).
AHN, a subsidiary of Mercy Health and a member of Trinity Health and the Trinity Health Accountable Care Organization (ACO), has committed to be among the first Next Generation ACOs within the new Centers for Medicare & Medicaid Services (CMS) model. AHN's clinically integrated network (CIN), including its Grand Rapids, Michigan region providers, will expand its rollout of Wellcentive's platform to solidify data aggregation and value-based care strategies throughout its network of primary and specialty providers delivering care in both practice and hospital settings, and coordinate its participation growth within alternative payment models.
"We have been working to support clinical changes to improve care in our community while enabling our providers to meet new quality requirements," said Affinia Health Network Medical Director David Van Winkle, M.D. "This expansion will further our ability to take advantage of new payment methodologies, redesign and improve care, and improve financial outcomes under these new payment models. Our population health strategy will bring value to both our patients and our providers, addressing routine, intermediate and complex care needs."
As part of the Trinity Health ACO, AHN providers will deliver care within CMS' Next Generation model along with patient-centered medical home and private payer programs. Next Generation allows organizations to participate in incentive-based payment approaches that reward accountability for outcomes, risk, and cost management of its population.
Added Van Winkle, "As we move into shared-risk or full-risk contracts, these value-based, patient-centered models and our increased ability to meet them will serve our members, our specialties and our region well."
"Affinia was an early adopter of Wellcentive's population health innovations, and our partnership has allowed us, together, to study, experience and innovate toward a comprehensive value-based care solution for the industry," said Tom Zajac, Wellcentive CEO. "This expanded relationship will support Affinia Health Network's strategy for longitudinal care coordination, patient and provider outreach, management of risk and financial outcomes throughout its market, and meet the needs of data aggregation and quality reporting under new payment models."
The ability for delivery networks to align population health management practices around patient engagement, referral management, risk stratification and data translation, positions them for emerging payment models such as CMS bundled payments expanding this year and the horizon launches of the agency's Comprehensive Primary Care Plus model, and the Medicare Access & CHIP Reauthorization Act of 2015 – programs historically mirrored by private payers.
