Patient-Centered Medical Home Program Expands to Allow All Physicians to Share Clinical Data on All Patients
ATLANTA, March 2 /PRNewswire/ -- MDdatacor, Inc., a leader in health care quality management solutions for insurers and health care providers, today announced the go-live of the statewide clinical quality health information exchange functionality of their MDinsight® application to support the Blue Cross and Blue Shield of North Dakota (BCBSND) MediQHome(SM) Quality Program. Clinicians and their support staff throughout North Dakota and Western Minnesota will have access to clinical quality data on all their patients, regardless of insurer.
Physician practices participating in the MediQHome(SM) Quality Program have been using the MDinsight® web-based portal since July 2009 to identify opportunities for additional patient care and ensure patients are treated according to best practices as outlined in clinical guidelines, such as identifying patients due for preventive screenings and chronic disease follow-up care. The MediQHome(SM) program is the first statewide patient-centered medical home program in the country that uses clinical quality data exchange between participating clinicians, enabled through the MDinsight® Health Information Exchange (HIE) functionality.
With the new quality data-sharing functionality, physicians are able to seamlessly and securely exchange health information with other physicians throughout the state. This means that a physician encountering a new patient can instantly access the patient's previous lab results, medication and treatment histories from other providers, as well as view gaps in care. The sharing of data improves care coordination, providing enhanced opportunities for clinical improvement and better patient care.

The MDinsight® system uses patented technology to automate the collection of patient clinical data from all of a physician practice's disparate data sources, including electronic medical record (EMR), practice management, lab, and registry systems -- even transcribed notes. Aggregating data from multiple sources provides physicians with the most comprehensive clinical patient information available.
