Managed long term care plans (MLTCP) are designed for people who are chronically ill or have disabilities which require more than 120 days of community-based long term care services. The Centers Plan helps its members and their families contact the medical providers that are right for them, both in coverage and location. The MLTCP also schedules and pays for a large selection of health and social services, including transportation to and from appointments, home health aides, social day programs, meals on wheels as well as many other benefits. To become a member of Centers Plan for Healthy Living, you can call toll free (855-270-1600) during regular business hours with any questions or concerns. Once all questions have been answered, a nurse will make a home visit to determine needs, explain the mechanics of the program, and how it can be most beneficial to members. Once eligibility has been determined and an application to the state has been approved, membership will become effective on the first day of the subsequent month.
