Centers Plan for Healthy Living Introduces New Medicaid Managed Long Term Care Plan for Local Community

NEW YORK, April 18, 2013 /PRNewswire/ -- Centers Plan for Healthy Living, a new Medicaid Managed Long Term Care Plan started enrollment activities February 1, 2013. The Plan's goal is to coordinate care to Medicaid recipients over 21 years old with chronic needs in the New York City metropolitan area and Rockland, Niagara and Erie counties. Managed long term care plans (MLTCP) are designed for people who are chronically ill or have disabilities which require more than 120 days of community-based long term care services. Once all questions have been answered, a nurse will make a home visit to determine needs, explain the mechanics of the program, and how it can be most beneficial to members. Once eligibility has been determined and an application to the state has been approved, membership will become effective on the first day of the subsequent month.
