Community Information Exchange San Diego Launches Pilot, Secures $1 Million Expansion Grant, and Becomes a "Live Well San Diego" Partner
SAN DIEGO, Dec. 9, 2014 /PRNewswire-USNewswire/ -- CIE San Diego has launched an innovative trust network and collaboration catalyst that provides the connective tissue that enhances care coordination among social service and care providers by enabling them to easily share client-level information; earned a second $1 million grant from the Alliance Healthcare Foundation to expand to organizations serving seniors aging in community and; will be recognized as a "Live Well San Diego" partner on December 11th 4-6p.
CIE San Diego today announced the go-live of one of the first social service information exchanges in the United States. Consistent with the National Institute of Medicine's (IOM) recent recommendation to include social and behavioral determinants in electronic health records (EHRs), the CIE network offers a more holistic view of a client's social history and makes it easier for case managers, intake/discharge coordinators to connect with other care providers. In its initial phase, CIE San Diego is sharing information among community based organizations, government, and healthcare organizations to enhance care coordination on behalf of vulnerable populations such as homeless individuals and seniors. Ultimately, CIE San Diego aims to strengthen the non-profit sector by creating a comprehensive social service information exchange that will facilitate inter-agency collaboration. By building the data sharing capacity of hundreds of diverse organizations, CIE San Diego hopes to serve as a model that can be adapted to other regions across the United States.
With more comprehensive information more readily available, nonprofit professionals can smooth care transitions more efficiently, perceive changes in client well-being more rapidly, and intervene with more collaborative programs—all of which can improve the client care experience while reducing system-wide costs and improving the health of populations. Start-up funding for CIE San Diego was provided by a 2012 Alliance Healthcare Foundation (AHF) Innovation Initiative (i2) grant and a second grant of $1 million was recently awarded to further expand the CIE San Diego network to seniors aging in community and to measure organizational and client-level outcomes. "Alliance Healthcare Foundation's goal in the Innovation Award program is to support initiatives that increase the capacity of local organizations through innovation and collaboration," said Rob McCray, Board Chair of Alliance Healthcare Foundation, "and we look forward to the time when all relevant agencies share data through the CIE San Diego network."
On December 11th from 4-6 PM , CIE San Diego will be designated a Live Well San Diego partner by Supervisor Ron Roberts and County of San Diego HHSA Director Nick Macchione. Launched in July of 2014, the CIE San Diego homeless pilot already enables more than 75 users across 6 organizations to access program history and case manager information for more than 800 homeless clients in downtown San Diego including:
- San Diego Fire-Rescue Department, Emergency Medical Services Division, Resource Access Program
- PATH/Connections Housing via Regional Taskforce on the Homeless (RTFH)
- Rural/Metro San Diego
- Scripps Mercy Hospital
- St. Vincent de Paul Villages
- UC San Diego Health System Hillcrest Medical Center
