Historically, it has been difficult for providers to identify, communicate and manage patient populations efficiently. Results from an American Hospital Association survey released earlier this month underscore hospital and health systems' increased adoption of technology designed to engage patients in their healthcare and enhance patient-provider communication. Representing more than 2,000 U.S. hospitals, forty percent confirmed their organization uses real-time care management services to provide virtual care for patients with chronic conditions.
