AiME makes it possible for patients and caregivers to interact in real-time with their personalized recovery plan, ask questions, manage medications, share images, request help with nutrition and exercise plans, and schedule virtual appointments with Dimer clinicians, all inside the company's clinical model. AiME is built to recognize when a concern requires human judgement and escalates directly to one of Dimer's licensed clinicians the moment it is needed.

AiME addresses one of healthcare's most dangerous blind spots: the period after a patient leaves the hospital. Unlike standalone AI chat tools that provide generic answers, AiME is grounded in each patient's discharge instructions and medical record. Ownership of the post-discharge recovery period belongs to Dimer's licensed Transitionist™ clinicians, taking responsibility for the gap in care that today largely goes unaddressed.

Since its rollout in late 2025, AiME has supported thousands of chats with patients during their recoveries. Dimer Health, a physician-led Transitional Care Medicine company that pairs licensed clinicians with AI-driven technology, is now making it broadly available to health systems and the patients they discharge. AiME builds on Dimer Health's broader transitional care infrastructure, which has demonstrated reductions in 30-day readmissions of up to 50% in specific patient populations and reductions in lengths of stay for patients needing to return to the hospital if recovery veers off track. And the feedback from patients has been overwhelmingly positive.

In some cases, success is preventing an unnecessary readmission. Dimer patients who do need to return to the hospital get there in a median of seven days, four days sooner than patients without that oversight, because AiME caught the problem while it was still developing. And when they are readmitted they're in for a shorter stay: a median of seven days compared to eleven days for non-Dimer patients, a benefit of earlier detection.

AiME helps by explaining medications, running a reconciliation check, and flagging potential discrepancies — like duplicate medications or the same drug listed at two different doses — for a clinician to confirm. When something needs a human judgement call, AiME escalates directly to Dimer's Transitionist Team, the on-call, physician-led licensed clinicians equipped to make it. AiME does not replace clinicians. It works alongside them, handling day-to-day questions and routine tasks while knowing exactly when to elevate a concern to a licensed clinician. That same triage frees providers to spend more time on patients whose cases require a clinician's judgement, not routine questions.

AiME's guidance goes beyond clinical insight, to help address whole-patient recovery needs. One patient was uncertain about what diet to follow to avoid complications and still get the nutrients she needed. Drawing on her recovery plan and medical history, AiME generated a recovery-friendly grocery list and suggested a meal plan she could follow, while the Dimer Transitionist Team monitored her progress.
