Evolent Health Partners with Torrance Health IPA and Torrance Memorial Integrated Physicians to Provide Risk Adjustment Solution and Services

WASHINGTON, Aug. 7, 2018 /PRNewswire/ -- Evolent Health (NYSE: EVH), a company providing an integrated value-based care platform to the nation's leading health systems and physician organizations, today announced it is providing Torrance Health IPA (THIPA) and Torrance Memorial Integrated Physicians (TMIP) with its Identifi^SM population health performance management platform and Risk Adjustment solution to help enhance performance across Torrance's network of 500 providers in the Los Angeles metro area. THIPA and TMIP collectively manage more than 20,000 Medicare Advantage and Next Generation ACO lives.

In addition, Torrance is working closely with Evolent to deploy new Patient Assessment Forms (PAFs) and gauge the expected value of risk coding gap closures.
