Gateway Health Plan® Enters Philadelphia by Introducing Medicare Assured® HMO SNP Product
PHILADELPHIA, Oct. 17, 2011 /PRNewswire/ -- Marking its first health system agreement in Philadelphia, Gateway Health Plan® (Gateway) announced today that it is expanding its Medicare Assured® HMO SNP product for those on Medical Assistance and Medicare, effective Jan. 1, 2012. Gateway started accepting eligible members for enrollment Oct. 15.
Gateway signed multi-year contracts with Albert Einstein Healthcare Network, Community Health System, Mercy Health System of Philadelphia and Temple University Health System, increasing Gateway's service area for the dual eligible population - those who qualify for both Medicare and Medicaid assistance- to 28 counties in Pennsylvania. The expansion will provide potential members with access to a wide network of 1,500 local healthcare providers, including community mental health and ancillary services.

Gateway's Prospective Care Management (PCM®) model of care, as well as its experience as a Special Needs Plan (SNP), played a large part in Gateway's expansion to Philadelphia. A SNP is a specialized Medicare Advantage Plan serving members on both Medicare and Medicaid with severe disability or chronic illness requiring a skilled nursing facility or home care. The contracting hospitals and physicians realized the importance of coordinated patient care and helping members achieve the social services needed for optimal health.
By improving access to our most vulnerable citizens, Gateway has exceeded national standards for customer satisfaction and quality service. Our Prospective Care Management Program (PCM®) proactively addresses the challenges faced by our members in accessing medical and social support services. PCM® is a proactive, holistic approach that addresses the Behavioral, Environmental, Economic, Medical, Social and Spiritual (BEEMSS(SM)) issues a member faces that may be barriers to care. Gateway also is actively involved with a Health Literacy Program, called Gateway HLPS, which aims to help underserved dual eligibles and the community better understand and navigate the healthcare system.
