Harmony Health Plan Associate Honored as Care Coordinator of the Year by the Illinois Association of Medicaid Health Plans
CHICAGO and TAMPA, Fla., Dec. 7, 2016 /PRNewswire/ -- WellCare Health Plans, Inc., (NYSE: WCG) announced today that Durwick Galloway, a field outreach coordinator for its Harmony Health Plan, Inc., was named Northern Illinois Care Coordinator of the Year by the Illinois Association of Medicaid Health Plans (IAMHP). Galloway received this honor in recognition of his hard work and dedication to serving Harmony's Medicaid members in Illinois. IAMHP issues two Care Coordinator awards annually, one in the northern region and one in the southern region.
IAMHP is an organization of health plans that participates in Medicaid managed care in Illinois. Its members, which include Harmony Health Plan, are committed to improving the health of Illinoisans through high-quality health care that effectively and efficiently meets the needs of patients.

Galloway and other Harmony care coordinators work face-to-face with low-income families and children, seniors, and people with disabilities and other complex medical needs to help them navigate the health care system to ensure access to the appropriate level of care in the most appropriate setting. For example, Galloway recently accompanied an elderly man to his first visit with his new physician to make certain the member received the necessary tests and referrals. Care coordinators develop collaborative care plans with physicians to manage members' chronic conditions and connect members to community-based organizations to address social service needs, such as food, housing and transportation.

Galloway joined Harmony in 2016. He earned a bachelor's degree in psychology from the University of Arkansas at Pine Bluff, a master's degree in social work from Loyola University in Chicago, and a Master of Business Administration from Roosevelt University in Chicago.
As of Sept. 30, 2016, WellCare and Harmony Health Plan serve approximately 210,000 members in Illinois, including 167,000 Medicaid members, 16,000 Medicare Advantage plan members and 27,000 Medicare Prescription Drug Plan members.
