Innovaccer Partners With Carematix to Emphasize Remote Patient Monitoring on the Innovaccer Health Cloud

The partnership will enable providers and healthcare organizations to proactively monitor and engage with high-risk and rising-risk patients, improving care plan adherence and patient wellness.
