LCMC Health in Louisiana Partners with Care-at-Home Provider to Improve Patient Outcomes and Care

LCMC and myLaurel will deliver post-acute, in-home care to reduce length of stay and to provide safe, high-quality care following hospitalization

NEW YORK, Sept. 4, 2025 /PRNewswire/ -- As part of its commitment to providing the best possible care for every person and parish in Louisiana and beyond, University Medical Center | LCMC Health will now offer high-acuity home-based care through myLaurel, an experienced provider of acute and transitional care delivered outside the traditional hospital setting.

Together, the organizations will provide patient-centered care at home, with a focus on better supporting patients who are frail, elderly, or medically complex through their first 15-30 days at home post an acute hospital visit. Those hospitalized within the University Medical Center | LCMC Health network may now continue their recovery at home, supported by myLaurel's clinical care team.

When clinically appropriate, this model reduces the need for extended inpatient hospital stays. Additionally, by transitioning eligible emergency department and observation patients to home-based care, the program helps alleviate capacity constraints, improves patient flow, and delivers care in a setting that enhances comfort, experience, and outcomes.

University Medical Center New Orleans (UMC), the largest hospital in the LCMC Health system and one of Louisiana's leading academic medical centers, is the first to implement this new model of care. As a vital social safety net provider for the city, UMC plays a critical role in caring for New Orleans' most vulnerable populations. The launch marks a significant step in advancing more equitable and efficient care by providing an at-home alternative that supports both patient recovery and hospital capacity as UMC continues to serve the New Orleans community.

Based on myLaurel's experience with other health systems, where the program has reduced hospital readmissions by up to 50% and shortened average length of stay by 1.5 days for the patient enrolled, Dr. Carty expects that the service will help to ease ED crowding, improve patient flow, and reduce boarding throughout UMC. myLaurel began serving eligible patients, as directed by their care teams, at the end of August.
