Innovative home-based Acute Care at Home™ program helped improve outcomes for more than 1,000 general medicine patients in Brooklyn

BROOKLYN, N.Y., Sept. 16, 2026 /PRNewswire/ -- Maimonides Health, Brooklyn's largest healthcare system, and myLaurel, a leading tech-enabled home-based acute and transitional care company, today announced the one-year results of their strategic partnership. Initially launched in May 2025, the collaboration represents a highly integrated clinical effort that has successfully transformed care delivery for Brooklyn's frail, elderly, and medically complex populations through myLaurel's Acute Care at Home™ program, expanding the ability to provide advanced medical care at home by a multidisciplinary care team.

Over the past year, the program has served over 1,200 patients in the comfort of their homes, with an emphasis on a patient population that carries a much higher baseline risk of readmission than the general hospital population. The partnership has equipped patients and caregivers with the necessary skills, tools, and resources for a safe recovery, driving clinical success that extends well beyond the formal 15- to 30-day care transition window through timely in-home clinical interventions, virtual care, diagnostics, and therapeutics.

Data from the first year of implementation shows improved patient outcomes, with a 43% reduction in 30-day readmissions compared to the standard medicine service line and an average patient Net Promoter Score (NPS) of 97, highlighting strong, positive patient satisfaction trends. This translates to more than 1,000 unnecessary readmissions avoided and 2.29 hospital bed days saved per patient. By reducing readmission rates, the initiative has the potential to improve patient outcomes while helping mitigate financial penalties associated with the CMS Hospital Readmissions Reduction Program (HRRP).
