Leavitt Partners and Forward Health Group Announce Comprehensive Primary Care Plus (CPC+) Collaboration with a Call to Action for Physician Groups & Payers Engaging in CPC+

SALT LAKE CITY, Aug. 10, 2016 /PRNewswire/ -- Leavitt Partners and Forward Health Group have announced a collaboration to support physician groups and payers to succeed in Comprehensive Primary Care Plus (CPC+), a payment initiative set forth by the Centers for Medicare and Medicaid Services (CMS).

Primary care medical practices must apply for CPC+ by September 15, 2016, and those applying for participation in Track 2 must apply with a Letter of Support from an approved Health IT Vendor, such as Forward Health Group.

Four striking features stand out about CPC+:

1. CPC+ is CMS' largest ever initiative to transform and improve how primary care is delivered and paid for;
