Seniors in Azusa, California will have a home in a new 96-bed care facility, which will offer meals, mental health, medical care, and case management services to address their urgent needs.
SACRAMENTO, Calif., Oct. 1, 2025 /PRNewswire/ -- Health Net, one of California's most experienced Medi-Cal managed care health plans, and the Centene Foundation, the philanthropic arm of Centene Corporation (NYSE: CNC), today announced a partnership investment of $7 million to support The Wesley Azusa Housing & Health Program. The program includes a 96-bed facility that will provide comprehensive wrap-around services for unhoused individuals over the age of 55 who experience ongoing medical care needs.

The 27,300 square-foot residential care facility, 204 W. Paramount Street, will serve as a permanent home while also offering a comprehensive range of supportive services. These services will include:
- Primary medical care
- Mental health services
- Substance use counseling
- On-site nutritious food
- Group therapy
- Psychiatry and medication management
- Case management
- Laundry services
- Daily activities, such as arts and crafts, singing groups, and gardening
- Licensed clinical social workers

Set to open in 2026, the Housing & Health Program will leverage collaboration between the Los Angeles County Department of Health Services, managed care providers, and other community partners to help identify clients experiencing homelessness and connect these individuals to the program for immediate intake.
