NexJ Connected Wellness Platform Selected By Dossia for New York Community-Based Health Promotion Initiative
NexJ products to be integrated in to Dossia's Personal Health Management System for a New York City Health Department pilot program
TORONTO, May 8, 2012 /PRNewswire/ - NexJ Systems Inc., (TSX: NXJ), a leading provider of next-generation enterprise customer relationship management solutions (CRM), today announced NexJ Connected Wellness has been selected by Dossia, a leading Health Management Platform provider, to be used in conjunction with Dossia's platform as part of the New York City Health Department pilot program spearheaded by The Primary Care Information Project (PCIP).
For the initial pilot, five church health ministries in Brooklyn will partner with PCIP to offer their congregants access to a Personal Health Record. The health ministry volunteers will use the NexJ Connected Wellness Platform to review and take action on health readings shared by congregants.
"Community-based health initiatives play an imperative role in improving the overall health and wellness in communities across the country," said Steve Munini, Chief Operating Officer of Dossia. "We're very excited to be working with NexJ to deliver a system to the New York City Health Department for the promotion of health within specific communities."
Elements of NexJ Connected Wellness Platform that will be integrated in the Dossia Personal Health Management System include Health Trackers which allow participants to track new blood pressure values and view their historical measurements; Individual and Group Progress reports which allows Public Health staff to view the success of the program on a day-to-day basis; and graphical Population Health Surveillance dashboards that allow population health analysts to evaluate the success of programs and develop new programs that target specific conditions and new communities.
"NexJ is dedicated to developing solutions that connect patients with a circle of care of doctors, health coaches, and family members so patients can effectively manage their own health and take proactive steps that promote wellness," said William M. Tatham, Chief Executive Office, NexJ Systems. "The Primary Care Information Project is a great example of how NexJ Connected Wellness can be used to engage individuals and their communities to take positive action to manage high blood pressure and other chronic conditions."
