Longitudinal care for joint, bone, muscle and low back pain, delivered with partners across orthopedics, rehabilitation and health systems

The program provides ongoing, evidence-based support for joint, bone, muscle, and low back pain, coordinating with a patient's existing providers to extend care between appointments.

The Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model is a voluntary CMS Innovation Center program that expands access to technology-enabled care between regular provider visits, while rewarding participating organizations for improving health outcomes for people with Original Medicare. Its musculoskeletal track covers chronic musculoskeletal pain, defined as pain lasting more than three months in the bones, joints, muscles or connective tissue, over a twelve-month care period. Beneficiaries join voluntarily, either on their own or through a referral from a clinician. RevelAi Health has been enrolling patients in the MSK track since July 2026.

Partners including Team Rehabilitation, Keystone Orthopedics, Western Rehabilitation Network, Alliance Physical Therapy Partners and Limber Health have embraced the model, integrating longitudinal virtual care with patients' existing musculoskeletal and physical therapy care.

MSK ACCESS and Understood Care are both ACCESS Participants. Understood Care provides additional patient advocacy services to beneficiaries in the program.

The MSK ACCESS program also draws from specialized expertise from national partners. Through a partnership with Own the Bone, the American Orthopaedic Association's bone health quality program, MSK ACCESS offers clinician support for bone health, addressing the osteoporosis and bone health gaps common among these patients. RevelAi Health has also partnered with the National Spine Health Foundation to deliver evidence-based, high quality spine health education to patients.
