Revolutionary Senior Care Delivery Model Awarded $4.5M NY State Grant
RESTORATIVE CARE GRANT AWARDED TO TELISTAT™
ALBANY, N.Y., Feb. 22, 2017 /PRNewswire/ -- The New York State Department of Health announced that a $4.5M grant was awarded to TeliStat™ for its innovative Restorative Care Unit Program. This revolutionary program, which integrates groundbreaking proprietary technology with multidisciplinary physicians and highly trained nurses, resulted in a profound reduction of 30-day hospital readmission rates to 5% or less, while serving more than 1,000 patients over the course of 18 months.
More than 20% of Medicare patients who are discharged from a hospital to a nursing home return within 30 days. The federal government estimates the cost of 30-day readmissions for Medicare patients alone at $26 billion annually, with more than $17 million of the costs being attributed to return trips that may not have been necessary with proper care.
The grant funding was included in the 2016-17 NY state budget as a way to evaluate the program over a 3 year period to analyze its effectiveness and make recommendations to implement restorative care programs statewide.

This will allow TeliStat™ to showcase their efforts statewide, becoming an effective exemplar for additional restorative care programs throughout New York."
TeliStat™ has entered into a strategic partnership with Marist College in Poughkeepsie, NY to use high performance computing and analytics to provide IT support, as well as product research and development for existing and future technology.

The Restorative Care Unit was piloted at Golden Hill Nursing and Rehabilitation Center in Kingston, NY.
