New Patient Care Management Solution Streamlines Scheduling and Patient Engagement with Dynamics CRM Online

The Dynamics CRM based offering is designed to streamline and automate the scheduling and patient engagement process for healthcare organizations. The CarePak solution allows healthcare teams to stay on top of a patient's care plan with an integrated calendaring system configurable to specific protocols. Administrators can assign tasks to care team members, track patient interactions or metrics, and create unique alarms to alert supervisors if action is needed.

The CarePak workflow utilizes Microsoft Dynamics CRM's integration with Healthcare ERP systems to provide clinicians with the most accurate outpatient information as well as the ability to create unique alerts and track specific data points to stem notifications when a patient could be at risk. Automated touch points are created to connect and engage with patients through the discharge process as well as maintain a full 360-degree view of patient cases through a customized web portal.

The system comes pre-loaded with a generic care plan which is configurable to the organization's specific guidelines and protocols. Care team members are assigned to tasks on a patient's care plan via integrated calendaring, and are subsequently alerted as actions from the care plan are due.

Interactions with patients are tracked through the system which can include notes and specific metrics such as weight or any other relevant details. Alerts can be set to go off for a supervisor if any action is overdue by a certain amount, for example, if a patient's weight breaks certain thresholds or the patient is a no-show for an appointment. This automated method allows healthcare organizations to streamline and automate scheduling and patient data pre- and post-discharge, ensuring greater accuracy and relevancy of patient engagement data and follow up.
