Senior Home Care's Winter Haven Office Beats National Averages in Patient Re-Hospitalization Scores
CLEARWATER, Fla., July 11, 2013 /PRNewswire/ -- Senior Home Care's Winter Haven office, a Medicare-certified home health care agency located at 37 3rd Street Southwest, is pleased to report patient re-hospitalization scores that significantly beat national averages.
The office's 30-day re-hospitalization rate for Medicare patients with congestive heart failure is 8.76%, while the national average stands at 24.7%. For all patients and all diagnoses (514 cases), its re-hospitalization rate is 8.75%, compared to the national average of 20%, reported by the Centers for Medicare and Medicaid Services (Medicare-only patients) for the period 4/1/12 through 3/31/13.
