Southlake Regional Health Centre Advances Care Coordination & Chronic Disease Management with NexJ Connected Wellness

NexJ Connected Wellness to be used to deliver personal care plans and health coaching to patients

TORONTO, Sept. 25, 2013 /PRNewswire/ - NexJ Systems Inc., (TSX: NXJ) a provider of cloud-based software, delivering enterprise solutions for the financial services, insurance, and healthcare industries, today announced Southlake Regional Health Centre (Southlake), a full-service hospital with a provincial mandate serving more than one million residents of York Region and South Simcoe, will enhance the delivery of their chronic disease programs with NexJ Connected Wellness.

NexJ Connected Wellness takes a new, people-centered, approach to healthcare.  The platform, and wellness solutions available through it, facilitates the delivery of health information to patients and empowers them to share the information with their health advocates.  The end result is to initiate care coordination and collaboration across the continuum of care so patients can achieve their health and wellness goals.

Southlake will utilize NexJ Connected Wellness to aggregate and normalize patient health information from multiple internal information systems, NexJ Transitional Care Management to deliver personal care plans to patients and NexJ Health Coach to provide personal health coaching services delivered by York University.
