USMD selects Lightbeam Health's Population Health Platform

IRVING, Texas, June 10, 2014 /PRNewswire/ -- Lightbeam Health Solutions announced today that it has contracted with USMD to deploy the Lightbeam population health platform to support USMD's CMS ACO population as well as several other at-risk reimbursement programs.

USMD currently utilizes EHR systems from multiple vendors to manage its ACO, Patient-Centered Medical Home and SeniorCare patient population. The Lightbeam Health platform will aggregate data from these systems into a comprehensive enterprise-wide data warehouse and it will analyze the data to deliver actionable information directly to the point of care in order to improve outcomes and reduce costs. USMD's providers will have access to the full suite of Lightbeam solutions, including:

- Care Management system
- Searchlight^tm risk stratification tools
- Predictive and retrospective analytics
