Van Dyk Deploys HeartWays Program At Montclair Facility; Hires Advanced Cardiac Nurse Practitioner
Deployment of HeartWays at Van Dyk Montclair and investment in advanced cardiac nurse practitioner emphasize commitment to specialized post-acute care and reducing unnecessary hospital readmissions
MONTCLAIR, N.J., July 17, 2013 /PRNewswire/ -- Van Dyk Health Care, Inc. announces the deployment of its HeartWays cardiac rehab program at its Van Dyk Montclair facility, along with hiring Nicole Cofini, RN, cardiac nurse practitioner to oversee the program. Cofini comes to Van Dyk Health Care from Hackensack University Medical Center, where she was a medical-surgical registered nurse and an intermediate cardio-thoracic registered nurse. She earned a masters degree in nursing from William Paterson University in Wayne, NJ; and a bachelors in nursing from Saint Peter's University in Englewood Cliffs, NJ.
At Van Dyk Montclair, Cofini will oversee the HeartWays program, northern New Jersey's only program dedicated to cardiopulmonary rehabilitation.

In her new role, Nicole Cofini will provide open, regular communication between patients, their physicians and the Van Dyk care experts. She'll also develop and implement comprehensive care plans with input from the patients' physicians and Van Dyk's on-staff cardiologists and oversee daily active cardiac rehabilitation for patients who've experienced the following conditions:
- Angina
- Coronary Angioplasty (Balloon angioplasty)
- Coronary artery disease (CAD)
- Coronary artery bypass grafts (CABG)
- Heart attacks
- Heart failure
- Heart surgery
- Implantable cardioverter defibrillator (ICD)
