MONTCLAIR, N.J., July 17, 2013 /PRNewswire/ -- Van Dyk Health Care, Inc. announces the deployment of its HeartWays cardiac rehab program at its Van Dyk Montclair facility, along with hiring Nicole Cofini, RN, cardiac nurse practitioner to oversee the program. Cofini comes to Van Dyk Health Care from Hackensack University Medical Center, where she was a medical-surgical registered nurse and an intermediate cardio-thoracic registered nurse. She earned a masters degree in nursing from William Paterson University in Wayne, NJ; and a bachelors in nursing from Saint Peter's University in Englewood Cliffs, NJ.
At Van Dyk Montclair, Cofini will oversee the HeartWays program, northern New Jersey's only program dedicated to cardiopulmonary rehabilitation.
"We began our HeartWays program at our Ridgewood facility and, based upon its success, are now proud to be deploying it in our Montclair facility under the clinical guidance of Nicole Cofini," said Robert Van Dyk, president and CEO of Van Dyk Health Care, Inc. "HeartWays opens up new doors for patients suffering with cardiac-related challenges, and gives them the option to rehabilitate safely and comfortably in a setting outside of the hospital. HeartWays is a perfect example of how we have strategically aligned our resources to achieve quality in all areas – forming deeper collaborations with hospitals, avoiding unnecessary rehospitalizations and creating the most positive patient experiences."
In her new role, Nicole Cofini will provide open, regular communication between patients, their physicians and the Van Dyk care experts. She'll also develop and implement comprehensive care plans with input from the patients' physicians and Van Dyk's on-staff cardiologists and oversee daily active cardiac rehabilitation for patients who've experienced the following conditions:
- Coronary Angioplasty (Balloon angioplasty)
- Coronary artery disease (CAD)
- Coronary artery bypass grafts (CABG)
- Heart attacks
- Heart failure
- Heart surgery
- Implantable cardioverter defibrillator (ICD)
- Pacemaker surgery
- Percutaneous coronary intervention (PCI)
- Valve replacement
"HeartWays provides a gateway for quality care at all of the critical stages of cardiac rehabilitation – before, during and after a cardiac episode," said Jackie Cousins, administrator of Van Dyk Montclair. "Our expert care team guides patients through the journey back to health by working in concert with their personal physicians, our cardiologists and advanced cardiac nurse practitioners. We've built in measures to ensure efficient follow-up and interventions at each step of the way. This level of oversight not only provides the utmost care for the patient but also reduces in efficiencies by avoiding unnecessary hospital readmissions."
