ATLANTA, Oct. 13, 2015 /PRNewswire/ -- Wellcentive, a leader in value-based care and population health management solutions, announced today that it will empower two new innovative initiatives in Pediatric Population Health Management: Seattle Children's Care Network (SCCN) and Pediatric Partners in Care (PPIC), both at Seattle Children's Hospital.
Through a three-year contract, Wellcentive will deliver its population health management and value-based care solutions to support SCCN, Seattle Children's recently developed Clinically Integrated Network; and PPIC, a program funded by a $5.56 million CMS Innovation Center Grant that aims to improve health outcomes and reduce costs of care for some 3,000 disabled children with multiple medical issues.
"We at Seattle Children's Hospital are true believers in uniting clinicians to deliver the best care and the best outcomes in the most cost effective ways," said Dr. Sandy Melzer, MD, executive vice president, networks and population health at Seattle Children's, which has specialty regional clinics in Bellevue, Everett, Federal Way, Mill Creek, Olympia, Tri-Cities and Wenatchee. "Our ability to do that lies within diverse, but well coordinated, care teams that have access to accurate, current patient data. We chose Wellcentive to help realize the vast potential of this initiative."
Wellcentive will capitalize on its deep experience with complex data integration and normalization, its comprehensive solution platform, and its proven care coordination and care management capabilities.
"Seattle Children's Hospital innovates as few others do," said Tom Zajac, Wellcentive's CEO. "These two new programs underscore the human, clinical and economic potential of value-based care in pediatric settings. We are honored to partner with SCH in their mission to ensure better health for children in the region; and to collaborate in delivering their innovative models for transforming the health process."
Wellcentive's solution will be deployed on the SCCN and PPIC work. Some of the targeted services are: care coordination; customized reporting and analytics; integration and normalization of clinical, administrative, and payer data sources; and tracking of improvements in various quality metrics.
