The guideline recommends circumcision for male patients whose phimosis is caused by lichen sclerosus and has not responded adequately to appropriate medical treatment, including prescription topical corticosteroids.

Understanding Adult Phimosis and Lichen Sclerosus

Phimosis occurs when the foreskin cannot be fully retracted over the head of the penis. In adult men, acquired phimosis may develop because of chronic inflammation, scarring, or underlying skin conditions.

Symptoms can include foreskin tightness, discomfort, painful erections, recurrent irritation, and difficulty maintaining hygiene. In some cases, more advanced scarring may contribute to urinary symptoms.

Lichen sclerosus is one recognized cause of acquired pathological phimosis. The condition can affect genital skin, producing inflammation, changes in skin texture, and progressive scarring. Some patients may also develop involvement of the urinary opening or surrounding tissues.

Not every case of adult phimosis is caused by lichen sclerosus, and not every patient requires surgery.

What the 2026 Clinical Guidelines Recommend

The updated guideline identifies potent or ultrapotent topical corticosteroids as standard medical treatment for genital lichen sclerosus.

For male patients who develop phimosis associated with lichen sclerosus and do not respond sufficiently to appropriate medical therapy, the guideline recommends circumcision, preferably with complete removal of the affected foreskin.

The recommendations also address continued medical management, examination of removed tissue, and appropriate follow-up care.

Importantly, circumcision may relieve foreskin-related symptoms without necessarily eliminating the underlying inflammatory disease in every patient. Some individuals may require additional treatment or monitoring after surgery.

These recommendations reinforce the distinction between circumcision performed for personal reasons and circumcision considered for an established medical indication.

"Clinical guidelines are valuable because they help clarify when conservative treatment may be appropriate and when a surgical approach deserves consideration," said Dr. Nourparvar. Additional 2026 Research Examines Patient Outcomes

A separate 2026 study published in Frontiers in Medicine examined treatment outcomes and quality of life among 55 men with genital lichen sclerosus.

Of those participants, 40% had undergone circumcision. Researchers reported improved quality-of-life measures associated with circumcision, particularly among men experiencing phimosis.

However, some patients continued to require topical corticosteroid treatment following surgery, underscoring the chronic nature of lichen sclerosus and the importance of ongoing medical evaluation.

The researchers acknowledged limitations, including the study's retrospective design, relatively small patient population, and differences in individual treatment histories. Consequently, the findings do not establish that circumcision will produce the same results for every patient.

Together, the guideline and clinical study highlight the importance of matching treatment decisions to the underlying diagnosis rather than relying on a single approach for all patients.

Informed Decision-Making for Adult Circumcision

Adult circumcision may be considered for persistent pathological phimosis, certain recurrent inflammatory conditions, and other established medical indications.

As with any surgical procedure, circumcision involves potential risks, including bleeding, infection, wound-healing complications, scarring, and changes in sensation. Patients should receive information about these risks, appropriate pain management, recovery expectations, and available alternatives before making a treatment decision.

Dr. Nourparvar emphasizes that men experiencing persistent foreskin problems should undergo an appropriate medical evaluation to determine whether observation, medical therapy, specialist assessment, or circumcision is warranted.
