In brief: Delivering high quality care while maintaining costs often hinges on the ability of physicians from separate specialties and physical locations to communicate. This can be especially true when patients visit a hospital or emergency department and rely on information flow between the hospital-based providers and their primary care physicians for a coordinated treatment plan. Without a continuity of care across physicians, poor outcomes, including costly readmissions, can rise. Axial Exchange and Alcatel-Lucent have teamed up to create a solution that allows care teams within hospitals to collaborate with outside physicians. The joint solution is comprised of the Axial Communication Platform and the Alcatel-Lucent MyTeamwork product.
The Axial Communication Platform enables hospital-based care providers to share information with outside providers. For example, when a patient is admitted to the hospital, the Axial system immediately alerts the relevant outside care providers. Then, Axial gathers clinical information from the various hospital systems and provides electronic summaries of the hospital visit to the various outside stakeholders. The clinical summaries are designed for the information needs of each stakeholder, such as primary care physicians, specialists, and medical home case managers. No special software is needed. The summaries are available on smart phones and web browsers and are delivered real-time as the information is captured within the hospital. As the Axial Communication Platform leverages both open source software and a cloud-based delivery model, it is available at a fraction of the cost of historical health IT solutions.
The Alcatel-Lucent MyTeamwork product provides seamless voice functionality within the Axial Communication Platform. For example, with one click, a primary care physician can collaborate with the care team within a hospital setting to provide key background information about a patient's clinical history.
