Under the agreement, Caradigm will enrich its population health solutions with the predictive clinical analytics capabilities of MEDai's solution, enabling customers to stratify patient populations according to severity, disease state, cost or risk profile. This will allow risk-bearing organizations to use their network of resources to focus care on those who will benefit most, helping create a positive impact on overall health outcomes and total cost of care. When coupled with Caradigm® Care Management software, providers can develop personalized care plans that specify problems, goals and interventions, and manage patients and conditions through timely interventions enabled by near real-time, event-based alerts.

Customers gain the following insight about populations and individual patients:

• A total risk score, with the ability to change the relative weighting of the scores that make up the aggregate score
• Patient stratification based on risk drivers, i.e., the specific elements causing a patient or cohort to be ranked at a certain risk level 
• The directional trend of the risk drivers for that patient or cohort (increasing or decreasing)
• Trends, e.g., medication adherence, that can be addressed across multiple patients
