The Central Jersey Mobile Care Transition Network™ powered by CareInSync's Carebook mobile platform will be used to track and monitor high risk patients discharged from six prestigious hospitals, including nationally-ranked Robert Wood Johnson University Hospital, Robert Wood Johnson University Hospital at Rahway, CentraState Healthcare System, Raritan Bay Medical Center, Saint Peter's University Hospital, and Trinitas Regional Medical Center. Carebook™ is a HIPAA-compliant, mobile care coordination platform connecting care providers across settings in real-time to deliver patient-centered collaborative care. Using Carebook, care team members can securely communicate across the team to collaborate on risk assessments and evidence-based transition plans. As patients transition out of the hospitals, they will get "picked up" by the network. Transition Coaches will then utilize Carebook to coordinate referrals from one of eight participating community-based organizations (CBOs): VNA of Central Jersey, Robert Wood Johnson Visiting Nurses, Community Visiting Nurses Association, Holy Redeemer Home Care, Monmouth County Division on Aging, Disabilities and Veterans Services, Middlesex County Office of Aging and Disabled Services, Union County Division on Aging, and Jewish Family Service of Central New Jersey. Aftercare providers from these CBOs will then use Carebook on their mobile tablets to follow the patients and administer the Care Transitions Intervention® program. Finally, Carebook's analytics dashboard will provide real-time visibility into care process measures and the ability to track the progress of the high risk patient population post-discharge.
