CareMore Health Plan Emphasizes Better Health through Preventive Care, Enhanced Benefits and Award-Winning Care
TUCSON, Ariz., Nov. 20, 2015 /PRNewswire/ -- As Medicare's primary sales season gets under way, CareMore Health Plan is emphasizing better health for Arizona seniors through award-winning Medicare Advantage plans with enhanced benefits and personalized care programs. CareMore Medicare Advantage (MA) plans continue to offer significant benefits to Arizona beneficiaries not available in Original Medicare, including preventive screenings and enhanced hearing and vision benefits.

Now in its sixth year in Arizona, CareMore offers various plans in the Tucson and Green Valley area of Pima County where it is one of the largest MA plans. In Maricopa County CareMore will offer a plan for those who reside in nursing homes and assisted living facilities.
The Annual Election Period, or AEP, began Oct. 15 and continues through Dec. 7. Plan selections made during that time are effective Jan. 1, 2016. Following are highlights on CareMore Health plans and programs available for election during the AEP period.
Disease Prevention Programs: CareMore has long provided a program to help older adults manage type 2 diabetes. Its new Diabetes Prevention Program was instituted to help address the more than 86 million Americans living with prediabetes. The program includes fitness services at Nifty After Fifty and features a mix of nutrition, weight loss, exercise counseling and group support.
Institutional Needs Plan (ISNP): As ISNP's continue to decline across the country, CareMore continues to lead the way for ISNP's in Arizona. In addition to its large membership in Pima County, the "Touch ISNP" plan is being reintroduced back into Maricopa County beginning 1/1/16. The Touch plan provides customized benefits and clinical delivery to the residents of senior living communities including assisted living and skilled nursing facilities.
Enhanced Telehealth Services for Disease Management: Since its start in 2006, CareMore's remote monitoring program has contributed to a reduction in hospital readmissions of nearly 42 percent among patients with chronic diseases. Patients living with chronic conditions are monitored at home using wireless devices that transmit data about their specific condition such as blood pressure and weight to CareMore clinicians. The program has been expanded to include patients with chronic obstructive pulmonary disease (COPD) and diabetes in addition to those with conditions such as congestive heart failure and hypertension.
Brain Health Program: The CareMore Brain Health Program is an early intervention program designed for patients with behavioral and psychiatric symptoms of dementia as well as their caregivers. The program takes a broad view of the issues confronted by patients with dementia including fall prevention, medication safety and adherence, legal challenges, and nutrition. Early results of the program show the program reduced emergency room visits, hospitalizations and falls in dementia patients. Started in California, the program has been extended to additional regions including Tucson, Arizona.
