Shared care planning and coordination involves creating a care plan, a personalized written statement of a care recipient's comprehensive health and social support assessed needs. It sets out what support providers and caregivers should deliver, why and when, and who is meant to provide it.
The ability to communicate with, and educate, patients in their home is an important feature for care management.
