WISDOM2ACT is designed to track changes, streamline and encourage early intervention and improve outcomes for older adults living in residential communities. WISDOM2ACT uses a simple workflow and easy to understand language to allow caregivers to use personal hand held devices to manage changes in residents' conditions quickly and easily.

WISDOM2ACT directly links the care team to clinicians in real time with four easy steps.
- Change Observed. A caregiver observes a change in a customer's condition—perhaps a change in gait or balance, or changes in personality or behavior which could be the sign of an adverse drug reaction or urinary tract infection.
- Change Documented. The caregiver documents the change as an alert, using a mobile device in real time. WISDOM2ACT is deployed through Eldermark Point of Care on a smartphone or other mobile device making the application readily available to all who interact with patients.
- Automated Alert to Nurse. The caregiver's alert is automatically routed to the nurse or other designated health professional—electronically, without the time lapses and missed messages that can come from paper-based systems. The alert remains active until resolved.
- Action. The nurse and care team follow-up with appropriate action, including conferring with pharmacists, physicians, and other clinicians.
