Group Practice Forum Launches Fromer-GPF Patient Journey Providing Physicians with Simple Solutions to Improve Patient Care

NEW YORK, July 8 /PRNewswire/ -- Group Practice Forum (GPF), an independent network of medical professionals, today announced the official launch of the Fromer-GPF Patient Journey, a proprietary workflow solution designed to provide primary care physicians with simple solutions to improve patient care.  The Journey was developed by the Executive Medical Director of GPF, Len Fromer, M.D., FAAFP, who also serves as Assistant Clinical Professor of Family Medicine at University of California at Los Angeles School of Medicine and Western University of Health Sciences.  

The Fromer-GPF Patient Journey is a team-based model that equips primary care physicians treating patients with chronic diseases with valuable tools that can be integrated into their practices immediately.  Using the Journey model, physicians and their patients can gain practical solutions to incorporate into a chronic disease care model; methods to improve patients' health literacy; improved quality of the patient-physician dialogue; and an enhanced journey to physician-supervised patient self-management.
