Hello Health Implements Reimbursable Chronic Care Management Telehealth Services to Support Remote Medicare Patients

Practices are successfully providing monthly care to patients while being fairly and fully compensated under code 99490

NEW YORK, Sept. 23, 2015 /PRNewswire/ -- Hello Health, a leader in Meaningful Use physician practice solutions, today announced the successful onboarding and reimbursement of practices using their outsourced Wellbox-Chronic Care Management System. With Wellbox, Hello Health partner practices will be in full compliance with the Medicare CPT 99490 Chronic Care Program, which rewards practices that provide specific non-face-to-face services to qualified patients over a calendar month.

Chronic conditions such as heart disease, stroke, cancer, and diabetes impact 117 million Americans and are among the most common and costly health problems to manage and treat. For the first time, Medicare provides full and fair reimbursement for the 'tradition' of non-compensated telephone care that physicians have long afforded their patients in support of clinical excellence. Studies show that regular clinical engagement keeps patients who suffer from chronic conditions healthy and out of the emergency department.  

Hello Health supports medical practices that are interested in extending the quality of care provided in the traditional office setting with outsourced clinical telehealth expertise. Hello Health provides clinical conversations by licensed professionals directly with practices' patients, allowing for the collection of key symptoms that often go overlooked in chronic condition patients.  Post-conversation, the practice's EHR record is updated so that the patient record fully reflects real-time information to be used for care coordination and individualized population health efforts.
