Home Care by Black Stone Notifies Individuals of Security Incident
DAYTON, Ohio, Sept. 25, 2017 /PRNewswire/ -- Home Care by Black Stone (Home Care) announced today that it is notifying individuals related to a security incident involving certain personal information.
On August 15, 2017, our Dayton, Ohio office was broken into by an unknown person who stole certain computers and other electronics. One of the computers that was taken was the computer that was used by job applicants to submit background checks. Individuals who applied to a position at Home Care from January 16, 2014 to August 15, 2017 may have had information from their background check application saved on the stolen computer. The information on the application may include first and last name, address, Social Security number, and driver's license number.
Home Care takes the security of personal information seriously, which is why it took immediate action. Home Care immediately notified law enforcement of the theft. Home Care is offering a complimentary one-year membership to Experian's® IdentityWorksSM to individuals whose information may have been on the stolen computer. If a person applied to a position from January 16, 2014 to August 15, 2017 please contact Home Care at 937-643-2422, Monday through Friday from 9:00 a.m. to 5:00 p.m. EST. to obtain a code to enroll in the Experian service.
Home Care regrets this incident occurred and apologizes for the inconvenience.
