Kaiser Permanente Care Management Institute and the National Center for Complex Health and Social Needs to Host Webinar on Innovative Approaches to Complex Care

OAKLAND, Calif., Jan. 10, 2019 /PRNewswire/ -- Kaiser Permanente's Care Management Institute will host a webinar with the National Center for Complex Health and Social Needs titled "Innovative Approaches to Complex Care." The hour-long webinar will take place January 16, 2019, and will focus on innovative best practices used in caring for individuals facing intersecting medical, behavioral, and social challenges.

What: Attendees will learn how Kaiser Permanente's Care Management Institute identifies health care delivery models that improve care for these patients through holistic treatment leveraging the resources of its integrated delivery system. Attendees will also learn about the National Center for Complex Health and Social Needs' Blueprint for Complex Care, which provides a common framework for collectively advancing this emerging field. Importance of cross-sector collaboration and data sharing to bring about new ideas and approaches
