HUNTINGTON BEACH, Calif., April 4, 2016 /PRNewswire-USNewswire/ -- Landmark Health, an industry leader of home-based medical care for patients with complex health needs, announced the enrollment of its 5,000th patient today.
Landmark Health was founded in 2013 with the mission of delivering team-based medical care to complex and chronically ill populations. Landmark clinical teams are physician-led and interdisciplinary in nature—these teams deliver comprehensive medical care to patients in the comfort of their homes, and are available for in-person consults 24/7. Landmark's clinicians partner closely with community providers, including primary care, to deliver collaborative and coordinated medical care for patients, many of whom have challenges accessing medical care with the frequency that their clinical profile demands.
Landmark Health conducted its first home visit on October 6th, 2014. Since that inaugural visit, Landmark has expanded its operations to encompass five urban markets in four states, including New York, Washington, Oregon and California. "At Landmark, we are focused on providing high-quality, comprehensive and compassionate medical care to our patients, wherever they reside and whenever they need it," said Adam Boehler, Chief Executive Officer.
Landmark's 5,000th patient was engaged in Seattle, Washington, where Landmark is delivering its program in collaboration with Premera Blue Cross and several leading physician groups and health systems in the city. Mr. Duane Grimmer, Landmark's 5,000th patient and a resident of Seattle, commented on the Program: "It gives me a great sense of security knowing that my Landmark medical team is here for me at all hours of the day and night. If I had Landmark as part of my care team earlier this year, I probably would have avoided going to the hospital, which would have saved me time, money and stress." Dr. John Espinola, Executive Vice President of Healthcare Services at Premera Blue Cross, describes the Landmark Program: "At Premera we are pleased to partner with Landmark. Landmark fills a critical need by delivering personalized and expert clinical services to our most vulnerable members. We believe that Landmark is helping to transform healthcare in our community."
To commemorate this milestone, Landmark Health has made a donation to the Pike Market Senior Center & Food Bank in Mr. Grimmer's name. Boehler further commented on Mr. Grimmer being named Landmark Health's 5000th patient, "It is a pleasure to serve Mr. Grimmer. We are proud to contribute to the local community, and we are looking forward to the positive impact of our medical model in the Pacific Northwest."
