PatientPing will alert NextLevel Health coordinators whenever their members have been admitted to a care facility so that NextLevel Health can support the safe discharge of their members.

PatientPing, a Boston-based care coordination platform, will provide NextLevel Health members with Pings, real-time notifications whenever one of their members receives care at any ED or facility within the PatientPing network. These notifications will include important context on the patient, including their updated contact information. PatientPing's network consists of tens of thousands of providers nationwide.
