$750,000 Grant to support implementation of a Population Health-driven strategy
PALM BEACH GARDENS, Fla., July 5, 2017 /PRNewswire/ -- Prestige Health Choice, a Medicaid managed care plan serving 55 of Florida's 67 counties and a joint venture between AmeriHealth Caritas and Florida Blue, recently honored Health Choice Network of Florida (HCNFL) for innovation in implementing a population health-driven strategy by awarding a grant of $750,000 in support of the strategy.
Presented by Prestige Health Choice's President Dwight Chenette at HCNFL's 23rd Annual Education Conference, the award highlighted HCNFL's commitment to supporting Federally Qualified Health Centers (FQHCs) in Florida on their journey to transforming health care delivery to focus on population health and improve health care outcomes and quality reporting. HCNFL's work supports Prestige's efforts to effectively improve value in the delivery of quality care by working jointly with its FQHC partners to provide services under value-based models of care that focus on prevention and timely access for all Prestige members so that they may achieve optimal health.

HCNFL Chief Executive Officer Alejandro Romillo accepted the award at the start of the conference, giving special recognition to the participating and committed community health centers.
