ANNAPOLIS, Md., July 2, 2014 /PRNewswire/ -- Reliance, a Southeast Michigan-based Accountable Care Organization (ACO) responsible for the coordinated care of Medicare participants, has selected the Synapse™ Care Management Application Suite from Synaptic Advisory Partners to power their population health management initiatives. Under the agreement, Synaptic will provide several population health management solutions to the entire Reliance network through its award-winning platform, Synapse - a cloud-based application built on the Salesforce.com platform which supports efficient, high-quality population stratification, patient coordination and care management.
Reliance initially selected a competing technology to address their needs, but after months of effort, they found they were not able to produce the data they needed to effectively identify and manage high-risk patients. Synaptic was contacted and ultimately chosen to support Reliance's population health management needs. "When we realized our previous platform just wasn't working, we contacted Synaptic as we had heard they had unique capabilities and could deploy their solution very quickly," said Gene Farber, Reliance COO. Within one week, they were able to take our raw CMS data, integrate those feeds into Synapse and deliver the customized reports we needed.
