TACOMA, Wash., Dec. 11, 2012 /PRNewswire/ -- Clinical leaders from Sound Physicians, a leading hospitalist organization, will share findings from the organization's multidisciplinary solution for decreasing readmissions at the 24th Annual National Forum on Quality Improvement in Health Care hosted by the Institute for Healthcare Improvement Dec. 9-12 in Orlando, Fla.
Presenters from Sound Physicians include:
- "Multidisciplinary Approach to Reduce Readmissions" will be presented by Elizabeth Monk , R.N., B.S.N. on Dec. 11. Monk is a hospitalist nurse at a Sound Physicians partner hospital in Michigan, one of several sites participating in the organization's readmissions pilot program. Her poster will be on display from 9 a.m. to 1:30 p.m. and 4:30 p.m. to 6:30 p.m.
- "Driving Quality Performance Across An Organization Utilizing Hospitalist RNs" will be presented Dec. 11 by Dianna Budgeon , R.N., B.S.N., Associate Director of Nursing at Sound Physicians. Her poster presentation is on display from 9 a.m. to 1:30 p.m. and 4:30 p.m. to 6:30 p.m.
In addition to the IHI presentations, Sound Physicians' director of clinical operations, Marina Farah , M.D., M.H.A., will present at the Academy of Healthcare Improvements 18th Annual International Scientific Symposium on Improving the Quality and Value of Health Care, held in conjunction with the IHI National Quality Forum.
- "A Systematic Multidisciplinary Solution for Decreasing Readmissions" will be presented by Dr. Farah on Dec. 11. Her storyboard will be on display from 9 a.m. to 1:30 p.m. and 4:30 p.m. to 6:30 p.m. on Dec 11.
