Telligen Wins Quality Innovation Network Award for Iowa, Illinois and Colorado
WEST DES MOINES, Iowa, July 30, 2014 /PRNewswire/ -- Telligen, Inc., a leader in population health management solutions, has been awarded the Centers for Medicare & Medicaid Services (CMS) Quality Innovation Network Quality Improvement Organization (QIN-QIO) contract for the states of Iowa, Illinois and Colorado. Charged with overseeing quality improvement efforts in these states, Telligen will support CMS' new approach to improving care for beneficiaries, families and caregivers.
"This role will allow Telligen to expand our population health management solutions by working directly with hospitals, nursing homes, physicians, home health providers and community and faith-based organizations on data-driven quality initiatives that improve the quality and safety of care delivered to Medicare beneficiaries and individuals dually eligible for Medicare and Medicaid services," says Jeff Chungath, Telligen CEO.
Each QIN‐QIO will work on strategic initiatives, such as:
- Reducing healthcare‐associated infections
- Reducing readmissions and medication errors
- Working with nursing homes to improve care for residents
- Supporting clinical practices using electronic health records to track and improve care management and care coordination
- Promoting disease management and prevention of cardiac disease and diabetes
- Reducing healthcare disparities through community, provider, and patient-centered care and services
- Improving patient and family engagement
"The positive impact QIN-QIOs will have when working in collaboration with national and local organizations is exciting," says Patricia Merryweather, Executive Director of Telligen QIN-QIO Programs. "Involvement in this contract will place Telligen at the forefront of transforming healthcare delivery by supporting learning systems, improving efficiency and value, and achieving better health outcomes for Medicare beneficiaries."
Over the course of the five-year contract beginning August 1, 2014, Telligen and 13 other QIN-QIO contractors will support the U.S. Department of Health and Human Services' National Quality Strategy and the CMS Quality Strategy to accomplish better care, better health for people and communities and affordable care through improvements. Telligen teams in each state will continue local work in conjunction with state partners, while pooling resources and common elements to best serve healthcare providers and Medicare beneficiaries across the region.
Prior to being awarded the QIN-QIO contract, Telligen held the QIO contract in Iowa for over four decades and in Illinois for over 15 years, leading dozens of CMS quality improvement initiatives focused on improving the quality, safety and value of care for Medicare beneficiaries. Telligen recently acquired a large majority of the healthcare quality improvement, data analytics and case review contracts held by the Colorado Foundation for Medical Care (CFMC). CFMC held the Colorado QIO contract since 1974 and also served as a national support contractor for QIOs in every state working with local communities to improve care transitions and reduce unnecessary hospital readmissions.
