AMGA launched a news and current affairs-style program in partnership with ITN Productions which explores how to change the way health care is being delivered from a reactive to a proactive care system.
'Transforming Chronic Care' aims to inform the sector and educate the wider public about the innovative ways of improving collaboration within the healthcare community, as well as of identifying those at high risk, preventing the onset of chronic conditions.
