The Diary® Releases Remote Patient Monitoring Module for Care Coordination
TUCSON, Ariz., Feb. 28, 2017 /PRNewswire/ -- Following successful implementation of initial pilots in Japan, The Diary Corp. has released a remote patient monitoring module as part of their care management platform, The Diary CarePro™. The complete cloud-based care management solution enables the effective delivery of remote care while helping care teams prove that they are meeting quality standards and decreasing cost.
CarePro's remote monitoring technology connects the patient and care team outside of a conventional healthcare facility, on a patient's personal device. The technology is designed for patients suffering from chronic diseases to better manage their conditions at home. CarePro automatically alerts providers and care managers to a change in their patient's condition. With remote access to health data, care teams can accurately assess critical needs, thereby mitigating the risk of emergency room visits.
The Diary's consumer-friendly patient app syncs with medical devices such as blood pressure cuffs and glucose monitors. The patient can easily dictate symptoms, as well as receive reminders to take medications. Caregivers and family members can access - the patient's health data and care plan through a mobile portal in order to assist with their self-monitoring and treatment adherence.
Preliminary pilot evaluation with Keio Medical University Hospital has proven an overall improvement of patient engagement. Over half of the elderly patients, between the ages 65-75, have chosen to continue the program. An engagement rate of over 50% far exceeds normal adoption rates among seniors.
This module is available on the Japanese Apple App Store in March and as part of fully integrated care solution in United States.
